At FTU we strive to provide the best and safest quality patient care by being highly stringent and meticulous with our Covid protocols. All staff, patients, and visitors are required to cooperate and work as a team to ensure the safety of everyone.
Proof of vaccination is no longer required to access our clinic
Masks are mandatory upon entering our clinic
Cloth face masks are not permitted and will be given a proper mask by an FTU employee prior to entering our clinic
Masks must be properly worn covering the mouth and nose by all FTU staff and visitors
Gloves and hand sanitizer are readily available
Visitors and patients will undergo a wellness check at our entrance in forms of:
Temperature check
Covid related questionnaire
Contact tracing form to be signed
Our waiting room will have a max of two chairs spaced at least 6 feet apart from each other
Our clinic is by appointment only. Patients and family members are to be asked to not arriveearlier than 10 minutes before their appointment
Upon arrival you may call 236-427-5029 and a FTU staff member will welcome you into our facility
Patients are allowed to bring up to two family members. They are to be seated at our waiting area
Chairs and pens used by visitors will be properly sanitized and wiped down by FTU staff after every session
All staff must perform proper hand washing procedure before and after every patient
All FTU staff and visitors must wear a proper mask in indoor common areas
Our clinic will be properly and routinely wiped down and sanitized at the end of the day
Examination room and equipment will be thoroughly sanitized after every patient
If any patient, family members, friends, or FTU staff is experiencing any flu-like symptoms must stay home and undergo testing

FTU staff members will assist you with rescheduling your appointment
Any missed appointments or cancellations within 24 hours will subject to a $50 fee
